At Sweet River 18ST SLS, Inc., we provide person-centered Supported Living Services designed in accordance with California DDS guidelines and Regional Center Individual Program Plans. Our goal is to ensure each individual receives the support they need to live safely, independently, and with dignity in their own home.

We work closely with families, service coordinators, and interdisciplinary teams to create and implement individualized support plans that reflect each person’s goals, strengths, cultural background, and preferences.

Our services include:

Independent Living Skills Development

Support with personal care, meal preparation, budgeting, household management, and daily routines to promote greater independence and long-term stability.

Community Participation and Inclusion

Assistance with accessing community resources, social activities, volunteer or employment opportunities, and building meaningful relationships.

Health and Safety Oversight

Coordination of medical appointments, medication support in accordance with Title 17 regulations, monitoring of health needs, and maintaining a safe living environment.

Behavioral and Emotional Support

Implementation of approved behavior support plans, crisis prevention strategies, and ongoing positive reinforcement to encourage growth and stability.

Advocacy and Compliance Monitoring

Active collaboration with Regional Centers to ensure services align with the Individual Program Plan, documentation requirements, and all applicable state regulations.
